Henry Ford Health System Community Health Outreach Worker - Health Alliance Plan in Troy, Michigan

GENERAL SUMMARY:

The Community Health Outreach Worker is responsible for providing telephonic and face-to-face interactions with members to support case management in coordinating services and providing linkages to community resources and will promote positive member experience and member retention.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Function as a part of an interdisciplinary care team to assist Care Coordinators with members with care coordination needs and improve the flow of member information.

  • Assist members eligible for additional services in a timely and accurate manner.

  • Obtain and review medical record documentation to assure Gaps of Care are addressed related to outstanding HEDIS measures

  • Identify and report clinical barriers to care and assist in effective outcomes.

  • Act as a liaison for members to support treatment adherence and care coordination, (e.g., PIHP, home health, community or facility-based services

  • Assist members with medical and social service appointments (specialist and/or physician appointment transportation and appropriate follow-up)

  • Assist with locating members who may be difficult to reach

  • Assist members to apply for and secure government subsides, services and community resources

  • Maintain a basic knowledge of state and federal government programs.

  • Maintain knowledge of community resources to assist members in accessing support services and develop and maintain a community resource database

  • Act as a community liaison between HAP Empowered and community organizations

  • Represent HAP Empowered in a professional manner, by providing excellent customer service.

  • Assist all HAP Empowered members selected for case management with identifying, securing, and sustaining resources and services to meet their health needs.

  • Other duties as assigned.

    EDUCATION/EXPERIENCE REQUIRED:

  • High School graduate or equivalent required.

  • Completion of a Medical Assistant Program or Nurse Aide Program preferred.

  • Associates Degree in a Health-Related Field preferred.

  • Minimum of two (2) years of recent experience (within the last three years) in a medical office setting required.

  • Experience working with Medicare and Medicaid programs and/or experience in related community services preferred.

  • Certified Medical Assistant or Certified Nurse Aid preferred.

  • Community Health Worker certificate preferred.
