Job Summary

Provides oversight and monitors staff activities for Home Care/Community Support Programs.

Duties and Responsibilities

  • Maintain current knowledge of Home Care/Community Support programs, services, policies and procedures to provide oversight of staff in accordance with applicable contract guidelines.

  • Provide staff orientation and supervision; coordinate staff assignments, monitor staff activities and review required documentation. Conduct employment interviews and make hiring/staffing recommendations for open positions. Prepare and conduct annual performance appraisals and corrective action discussions as needed with Director.

  • Assign staff schedules and daily/weekly tasks.

  • Assist in opening new cases, billing, and filing of required documentation.

  • Coordinate with funding source, the development, implementation and evaluation of Person Centered Care Plan in coordination with Director.

  • Complete initial Home Environment Assessment and continue assessments on an on-going basis.

  • Review documentation included in Person Centered Plans, Care Plan, and Confirmation of Services to assure compliance; document and report inaccuracies or discrepancies to Director.

  • Maintains 'on call' availability as necessary; respond to pager and/or phone calls; follow up with Director to take appropriate action when necessary.

  • Assist individuals with activities in daily living including but not limited to household tasks, communication skills, social and recreational skills, financial management, and activities in the community.


Job Qualifications

Education, Training, and Licensure/Certification

  • Associate's degree in human services, or equivalent education and experience.

  • Bachelor's degree preferred.

Experience

  • Three years working with individuals with developmental disabilities, mental illness and/or senior adults in a residential setting.

  • One year supervisory experience in a related human service field preferred.

Knowledge, Skills and Abilities

  • Strong knowledge of social service administration/management and staff supervision.

  • Ability to apply principles of logical thinking to a wide range of intellectual and practical problems.

  • Advanced ability to read and interpret legal, technical, and financial documents.

  • Ability to work creatively with staff, physicians, families and superiors.

  • Ability to display compassion and sensitivity with vulnerable adults who may become angry, hostile, or combative.

  • Ability to communicate effectively with individuals with varying levels of cognitive and physical impairments.

  • Ability and willingness to work constructively with individuals from diverse cultural backgrounds.

  • Ability to maintain a helping role and intervene with client care when appropriate.

  • Ability to communicate expectations clearly, and to effectively utilize a teamwork approach with staff at all levels.

  • Ability to add, subtract, multiply and divide all units of measure.

  • Ability to read, write, speak and understand English language.

  • Writes legibly with proper use of spelling, grammar and diction. Ability to work independently and with minimal supervision.

  • Ability to handle multiple tasks simultaneously.

  • Knowledge of and willingness to utilize basic computer software applications.

Additional Work Requirements

  • 24/7 availability required with the ability to work extended hours, evenings and weekends if needed. On-call responsibility for assigned area required.

  • Valid driver's license with good driving record; reliable ongoing personal transportation suitable for safely transporting clients; current automobile registration and proof of automobile insurance.

Physical and Mental Requirements

  • Ability to lift persons served weighing an average of 150 pounds.

  • Ability to transport persons in a wheel chair weighting up to 250 pounds.

  • Near and far visual acuity.

  • Hand-eye coordination.

  • Ability to move freely.
