With 30 programs, we offer treatment, prevention, growth and education services to more than 23,400 people annually. dir="LTR" align="LEFT"We have an exciting opportunity for a Care Liaison in the Adult Behavioral Health (ABH) Program to join our team of talented professionals. dir="LTR" align="LEFT"As a Care Liaison in the ABH Program, you will be responsible for:ulliConduct case management assessments to identify areas of need and service prefer Assesses the healthcare, educational, and psychosocial needs of the patient/family./liliCollaborates with PCP, patient, and members of the health care team, to assess patient, develop and implement an agreed upon plan./liliProvides self-management support and empowers the patient to achieve optimal health and independence./liliGather and integrate information from all available sources, including consumer self-reports, reports from family members, guardians and Adult Foster Care providers, primary care providers, Integrated Care Organizations documented medical and treatment history, needs surveys, assessments from other disciplines, etc.;/liliProvides follow up with patient/family when patient transitions from one setting to another. Completes post hospital discharge calls: Medication reconciliation, PCP or specialist follow up appointment, assesses symptoms, teaches warning signs, coordinates care, reviews discharge instructions, and problem-solves barriers./liliDemonstrates excellent written, verbal, and listening communication skills, positive relationship building skills, and critical analysis skills./liliParticipates in continuous quality improvement to enhance care management in the office setting./liliMaintains required documentation for all care management activities./liliUtilize motivational interviewing techniques to assess and articulate the motivation of consumers to address specific needs identified during the assessment process;/liliAssist consumers in determining their eligibility for entitlements, community services and supports;/liliProvide consumers, family members and guardians with complete and accurate information regarding services, supports and other resources available to meet needs identified during the assessment process;/liliConsider and promote the utilization of group and support services;/liliAssist with the development of comprehensive and integrated Individualized Plans of Service with consumers and other members of the treatment team (consistent with principles of Person-Centered Planning, Integrated Dual Disorder Treatment, Self-Determination and current Medicaid Guidelines);/liliProvide face-to-face interventions to each assigned consumer at least monthly;/liliConduct home visits and community outreach as needed to help engage consumers, identify needs and barriers, and assure access to services and supports;/liliLink consumers to community services and supports based on the needs and preferences identified in their Individualized Plans of Service;/liliConduct skill building interventions with individual consumers and groups of consumers as indicated in their Individualized Plans of Service;/liliMaintain regular contact with treatment team members and other providers to monitor and coordinate service delivery efforts;/liliReassess the service needs and preferences of consumers as needed, at a minimum every six months;/liliCoordinate the client’s primary and other health care providers to assure continuity of care./liliCoordinate and assis
