PACE Southeast Michigan Nursing Manager in Southfield, Michigan
POSITION SUMMARY:Under the general supervision of the Director of Clinical Operations, the Clinical Nursing Manager is responsible for operational over-site clinics and RN Case Managers. In addition, the Clinical Nursing Manager is responsible for facilitating specified Interdisciplinary Team meetings. SPECIFIC DUTIES AND FUNCTIONS: bull; Supervises and coordinates all the clinics at the PACE Southeast Michigan (PACE SEMI), as well as supervises licensed personnel in the Case Management and Home Care Department. bull; In collaboration with the Center Manager, facilitates the Participant-Centered Planning meetings, as well as the Care Plan Development/Enrollment meetings, to assure consistency in care planning and service authorization for participants enrolled at PACE SEMI.bull; Assists the Medical Director as requested in duties related to clinical and operational issues and special projects as may be required. bull; Communicates with the Director of Clinical and Center Manager regarding case management/homecare operations and issues. bull; Facilitates administrative, quality and performance improvement requirements, such as reports and statistical records. bull; Participates in and/or facilitates Quality Assurance projects resulting from data results. bull; Ensures adherence to departmental and external standards in the provision of quality focused care by attendance at professional meetings/committees and review of national standards of practice. bull; Ensures adherence to budgetary guidelines. bull; Other duties as assigned. GENERAL REQUIREMENTS:bull; BSN required, master preferred. bull; At least five years of clinical long-term care experience, including one year with frail elderly population, and five years of supervisory experience. bull; Must have the ability to lead and direct other licensed nursing staff in the delivery of care, only acts within scope of his/her practice. bull; Must meet a standardized set of competencies (approval by CMS) before working independently. bull; Must be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact. bull; Must meet Registered Nurse State Licensure requirements (be legally authorized to practice in Michigan, and only act within the scope of his/her authority to practice). bull; Excellent oral and written communication skills necessary; must have strong interpersonal skills.
