ProMedica Health System RN / Registered Nurse - Case Manager - Hospice - Full Time - Days in Monroe, Michigan

The RN, Case Manager (Hospice) provides clinical management of an assigned caseload of patients. The RN, Case Manager (Hospice) plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interacts with patients, significant others, and other interdisciplinary team members while maintaining standards of professional nursing and clinical competency. The candidate will work with the interdisciplinary team to ensure patient and family physical, emotional, and psychosocial goals are met. The RN, Case Manager (Hospice) works to maintain the comfort and quality of life for those who are diagnosed with terminal diseases and must be capable of compassionate communication with patients and their families. The RN, Case Manager (Hospice) must have keen observation skills, high ethical standards, and knowledge of when to alert doctors and interdisciplinary teams about changes in patient conditions. This position is ideal for a candidate that has an emotional and physical ability to deal with the challenges and rewards of caring for those with severe illnesses. Job Requirements:

  • Current license in state of Michigan.

  • BSN preferred

  • Three to five years of clinical experience.

  • Current CPR certification.

  • Combination of homecare/hospice clinical and case management knowledge preferred.

  • Require effective interpersonal skills.

  • Excellent organizational/problem solving skills

  • A team player, flexible, and able to handle multiple tasks

  • Strong customer and team orientation is essential

  • Willing and able to learn computer skills
