About this role
The Clinical Coordinator is responsible to the H.S.A./Program Manager for the completion of various office clerical assignments. Coordinates provider and clinic schedules. Manages telehealth schedules. Utilizes ERMA-CM and PTRAX for all consultative services both on-site and off-site. Interrelates and works effectively with all levels of authority.
What you bring to the table
Education:
- Graduate from an Accredited School of Nursing.
Experience:
- One(1) or more years clinic Experience preferred.
Licenses/Certifications:
Current Licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state in which you work.
Current CPR Certification.
What you will do
Receive and direct all incoming calls on a multi extension telephone. Assure messages are accurately relayed to staff when they are away from the office. Bring messages to the appropriate desk
Receive, process and schedule all consultations
Track/report all off-site emergency procedures and inpatient hospitalizations.
Maintain and distribute all on-site and off-site clinic schedules
Inputs information into ERMA-CM and PTRAX
Distributes approvals/denials/alternative treatment/info requested recommendations for specialty and off-site referrals to appropriate destinations as requested. Responds as appropriate.
Creates and maintains weekly trip sheets and packets
Checks for pending requests – nothing in queue for greater than 7 days
Maintain a filing system. Make necessary updates to files, and modify filing system to accommodate changes in the conduct of the business affairs of the office
Schedules all appointments; notifies H.S.A. of any appointment that cannot be scheduled on-site within 30 days and offsite within 60 days
Reconciles monthly events detail report
Creates Sick Call, Physical and Chronic Care lists
Run weekly care management tracking log audits for deficiencies
Perform clerical responsibilities as needed
Perform other duties as assigned.
