In collaboration with the leadership team, is responsible for the analysis, planning, implementation and evaluation of the program. Collaborates with the Healthcare team to develop goals related to the development and management of these programs. Ensures the evaluation targets are met and properly recorded and reported.
Enhances and expands programs while developing and directing services and initiatives for the best interest of the targeted population. Builds, develops and promotes a variety of projects, activities and services in collaboration with the Healthcare team to address appropriate issues.
Provides education for patients and their families.
Assists with collaborative research including data collection, study design, maintenance of databases, grant writing, statistical analysis, presenting at national conferences and manuscript writing for the program.
Responsible for providing oversight for staff as required. Performs clinical assessment as designated by the nature of the program.
Participates as an active member on hospital/health system/community committees.
Required Associate's Degree related field
Preferred Bachelor's Degree related field
