Henry Ford Health System Nursing Coordinator (RN)- Ambulatory- Gastroenterology- Detroit Campus- Full Time, Days in Detroit, Michigan

GENERAL SUMMARY

Reports to the Nursing Leader/Administration. The Nurse Coordinator is a Registered Nurse who under the direction of department leadership develops, implements, monitors and evaluates a program of care for patients. Coordinates care across the continuum to facilitate quality outcomes. Collaborates with other healthcare team members including physicians, nurses, extenders, ancillary and support staff to ensure optimum patient care and satisfaction. Provides education to patients and family regarding disease process. Trains and directs a nursing team in the optimal care of the patient.

PRINCIPALS DUTIES AND RESPONSIBILITIES

Leadership Duties:

  • Trains, directs, and supervises nursing team to accomplish established goals of department.

  • Coordinates with ambulatory nursing to develop nursing competencies for department.

  • Develop and operate process for both internal and external referrals.

  • Participates in activities that support accomplishment of the strategic plans and marketing/communication goals of the department.

  • Serves as an educational resource in clinical area of expertise. Coaches and mentors staff to achieve a higher level of practice through creation of a positive learning environment.

  • Designs and provides an educational program to address staff development and includes identification of desired learning outcomes/objectives based on systematic needs assessment, direct observation, and feedback.

  • Validates competencies of staff.

  • Works collaboratively with providers to provide the Henry Ford experience.

Physical Assessment

  • Recognizes significant and common abnormal lab results and changes in health status, and takes appropriate action.

  • Performs physical assessment on patient as determined necessary and/or designated by a physician/physician extender.

  • Performs rounds to evaluate patients.

Psycho-Social Assessment

  • Performs assessment of: health habits (smoking, alcohol use, dietary, drugs),safety issues (safety of home, driving, seat belts); availability of support systems (family network, coping mechanisms).

  • Observes and reports emotional stability of patient to physician/physician extender.

Diagnosis

  • Develops and evaluates nursing patient care treatment plans, based on nursing diagnosis.

Clinical Practice

  • Seeks to understand and incorporate both safe practices and evidence-based practices into daily nursing care.

  • Directs and delegates nursing care based on patient acuity and staff competency that is consistent with the nurse’s obligation to provide optimum patient care.

  • Schedules patients for special tests, clinic appointments, and follow-up care.

Consultation

  • Initiates social services and Home Health Care referrals.

  • Resolves patient complaints.

  • Acts as a liaison intra-departmentally to coordinate patient support services i.e., physical therapy, social work, radiology, general surgery, pathology, and any other identified service.

  • Collaborates with family and community resources for patient home care, upon direction or within a protocol.

Quality Assurance

  • Participates in departmental/divisional Quality Assessment and Quality Improvement Programs.

  • Maintains compliance with regulatory standards regarding practice, environment and documentation.

Research

  • Abstracts medical information from various sources in the patient medical record for inclusion in registries, research, quality assurance and utilization review.

Education

  • Designs comprehensive learning methodologies and strategies for patient/family education and provides mechanism for continuous feedback.

  • Enhances personal professional development through the review of current literature, attendance at educational programs, and participation in professional organizations in area of nursing practice.

  • Provides education for staff, colleagues, patients and/or families.

Other

  • Upholds goals, policies, procedures, and expectations of Henry Ford Medical Group (HFMG).

  • Maintains established departmental policies and procedures, objectives, Quality Assurance Program, safety, environmental and infection control standards.

  • Supports the mission, vision, and values of HFMG.

  • Performs other related duties as required.

EDUCATION/EXPERIENCE REQUIRED

  • Graduate from an accredited school of nursing and current licensure by the Michigan State Board of Nursing as a Registered Nurse.

  • Successful completion of HFHS (Parent Operating Group) Pharmacology test.

  • Maintains and demonstrates current clinical knowledge in area of specialty.

  • Bachelor’s Degree in Nursing preferred.

  • A minimum of three to five years of progressively more responsible clinical nursing practice in specialty.

  • Certification in specialty preferred.

  • Clinical knowledge and analytical abilities necessary to formulate effective nursing care plans.

  • Ability to perform a wide variety of professional nursing services.

  • Ability to effectively communicate with patients, families, and medical group personnel.

  • Preferred experience in the specialty.

  • Basic computer knowledge and keyboarding skills.

  • Knowledge of Word and Excel
